Patients can be referred by their GP via Team Care Arrangement which is designed to provide specialised individual treatment services to patients who have a chronic medical condition and complex care needs.

All programs maintain a focus on educating the individual to self manage their condition over the long term.

A chronic medical condition is one that has been or is likely to be present for at least six months.

These include a range of commonly preventable health conditions such as:

  • Chronic Respiratory Disease & Asthma
  • Diabetes & Pre-Diabetes
  • Cardiovascular Disease
  • Back & Neck Pain
  • Chronic Fatigue
  • Cancer
  • Depression
  • Osteoporosis
  • Physical restrictions or mobility issues
  • Obesity
  • Athritus
